Chronic Hives (Chronic Spontaneous Urticaria / CSU)

Chronic hives, also called chronic spontaneous urticaria (CSU), can be painful, unpredictable, and exhausting. Many patients deal with itching and swelling for months or even years without clear answers. Some are referred between allergists and dermatologists, while others simply wait for symptoms to fade.

With Tono Health, you can see board-certified dermatologists who specialize in chronic skin and allergic conditions, often within days. Through secure telehealth visits, you’ll receive a clear diagnosis, personalized treatment plan, and ongoing support, all from home.

What Are Chronic Hives (CSU)?

Chronic hives occur when the body’s immune system becomes overactive, releasing histamine that causes recurrent itchy welts, redness, and sometimes swelling (angioedema). Unlike typical allergic hives, CSU flares can appear without a known trigger and may last for weeks, months, or even years.

Symptoms of Chronic Hives

  • Itchy, raised welts or bumps that appear daily or frequently for six weeks or more

  • Red or skin-colored patches that may merge together

  • Swelling of the eyelids, lips, hands, or feet (angioedema)

  • Flare-ups that worsen with stress, pressure, heat, or certain foods or medications

Who Gets Chronic Hives

  • Adults and children, CSU can affect anyone, though it’s most common in adults aged 20–50

  • Women are slightly more likely to experience chronic hives

  • People with other autoimmune or thyroid disorders have a higher risk

  • Those with a history of allergies or asthma may also develop CSU

Common Triggers and Risk Factors

In most cases, CSU is not caused by a true allergy, but rather an immune imbalance. Possible contributors include:

  • Autoimmune conditions (thyroid disease, lupus, rheumatoid arthritis)

  • Stress, illness, or infection

  • Hormonal changes

  • Certain medications (NSAIDs, antibiotics, blood pressure medicines)

  • Heat, pressure, or friction on the skin

Why Early Treatment Matters

Chronic hives can greatly affect sleep, comfort, and quality of life. Without proper care, the constant itching and swelling may lead to fatigue, anxiety, and missed work or school.

Early evaluation by a dermatologist experienced in CSU can help:

  • Identify and manage triggers

  • Control itching and swelling quickly

  • Prevent flare-ups through long-term treatment plans

  • Avoid unnecessary allergy testing or medication cycles

Modern treatment options — including advanced antihistamines and biologic therapies— can bring significant relief, even for patients who have struggled for years.
