What causes radiation dermatitis?

Radiation damages the rapidly dividing cells of the skin in the treatment field. Severity depends on the total dose, the area treated (skin folds like the underarm and under the breast are more prone), concurrent chemotherapy or targeted therapy, and individual factors such as skin tone, smoking, and diabetes.

What are the stages of radiation dermatitis?

Grade 1 is faint redness, dryness, and mild itch. Grade 2 is brighter redness with patchy peeling, usually in folds. Grade 3 is moist, weeping skin over larger areas with pain. Late effects, months to years after treatment, include darkening, thickening, and small dilated vessels in the treated skin.

How is radiation dermatitis treated?

Gentle cleansing and bland emollients from the first day of treatment; topical steroids to control inflammation, which studies show reduce severity; barrier films and specialized dressings for moist areas; pain control; and prompt treatment of any infection. Care is coordinated with the radiation oncology team so treatment is not interrupted. Late changes can be treated with topical agents and laser.
